E-forms

If you work abroad you generally become no longer insured for general social facilities in ones own country. This can be properly arranged with the necessary E-forms. E-forms aim to facilitate cross-border contacts with and among the various social security bodies of the EER member-states. This means that you can use this form to transport your accrued rights from and to EER member-states.

There are different types of E-forms:

The E-100 series concerns benefits for sickness, pregnancy and secondment; The E-200 series for calculating and paying pensions, dependents' benefits and invalidity pensions; The E-300 series is used for all matters relating to unemployment benefits; The E-400 series is for entitlement to child benefit.

These forms are available from the various bodies in the own country.

An overview of the most common forms:

E-form Purpose To be obtained from
E101 Declaration of Secondment; is issued when a company seconds you to another country for a period of less than 12 months. This is accompanied by the E 111 Social Insurance Bank (SVB), International Secondment Department. For Belgium; Belgian Affairs Office, for Germany; German Affairs Office
E102 Extension of secondment via relevant body abroad
E104 Proof of insurance with compulsory health insurance in own country to avoid waiting times during long-tern stay in other member-state via social insurance implementation body (uvi)or Social Insurance Bank (SVB)via compulsory national health insurance (ziekenfonds)
E111 Proof of health insurance for short-term stay in other member-states, also needed for secondments via compulsory national health insurance (ziekenfonds)
E119 Proof of entitlement to sickness and maternity benefits in other member-state, to be accompanied by E 303 via social insurance implementation body (uvi), via compulsory national health insurance (ziekenfonds)
E301 Insurance periods of history of employment in member-state via social insurance implementation body (uvi), via compulsory national health insurance (ziekenfonds)
E303 Export of employment benefits for a period of no more than 3 months, to be accompanied by E 119
